Elder care planning services:
An experienced professional Geriatric Care Manager with knowledge of
local resources will meet with you to evaluate your long term care situation from an
objective point of view, providing you with a road map for the terrain ahead. We'll discuss the challenges, identify the strengths and resources from which the family can draw, and help identify
solutions with a written summary of recommendations.
Care Management services:
Working with a Care Manager on an ongoing basis, elders and their
families establish a relationship of trust with a professional upon
whom they can rely as other circumstances inevitably change. The Care
Manager will check in with the person on a regular basis to ensure
their safety and well-being, and can identify changes which may
indicate problems early, before they become crisis situations.
Professionals involved in the elder's care can be confident that
recommendations will be followed, appointments will be kept, and that
they will be kept informed. If in-home care is required, the Care
Manager can assist with selecting appropriate caregivers, and will
monitor the services to ensure that quality care is being provided.
Caregiver services: Elder Care Guides can work with families requiring in-home assistance to determine your needs and make the arrangements that will best fit those needs. We can coordinate services through local in-home care agencies, or recruit, screen, and hire care providers to be employed by Elder Care Guides. We believe that professional caregivers who are employed by an agency and work as a team with a professional geriatric Care Manager deliver the best outcomes for our clients, but for those who wish to hire a caregiver privately, we will work with you to ensure that the necessary screens and administrative tasks have been completed.
Placement services:
Because Elder Care Guides does not have contracts or referral
agreements with any residences, facilities, or ancillary service
providers, we are uniquely suited to assist families or professionals
by conducting an objective assessment of placement needs, and providing
appropriate referrals. The Care Manager will:
Meet with the client at their current residence, to assess their care needs and preferences related to living arrangements
Research residences, communities and/or facilities within the appropriate geographic area that meet these criteria
Provide at least three written recommendations
Additional
services that may be provided include: booking appointments to
tour/visit communities that have been recommended, referrals to
ancillary service providers as necessary (legal, financial or real
estate experts; moving services, senior relocation experts, etc.),
assistance with move-in paperwork, and a post-move follow-up call. The
cost of our placement service is a flat fee of $450.
